Hope Family Care Center is committed to providing the residents of Kansas City’s east side with personalized, quality healthcare. We accept all patients, regardless of their ability to pay, and address the needs of their total well-being.
Our service area is comprised of zip codes 64109, 64127, and 64128. In our community, hunger is real, transportation is a challenge, and life expectancy is lower than in any of our adjacent areas.
In our east side community:
- 16% are unemployed
- 14% have transportation issues
- 10% deal with food insecurity
- 7% are homeless
Located at 3027 Prospect Ave., Kansas City, MO 64128, we are currently the only nonprofit primary care facility available to our community. With your help, our patients who face significant social, economic, and environmental challenges will have access to proper healthcare and a better tomorrow.
Easy Ways to Make a Donation
Donations, fundraising, and contributions are our lifeline here at Hope Family Medical Center. We have two ongoing initiatives, each aimed at nourishing the complete self, the family, and long-term health habits.
Donate to A Mother’s Hope
Being a new mom can be overwhelming! Our initiative, A Mother’s Hope, supports moms with newborns by providing supplies to help with care at home. Monetary donations are accepted, and we welcome items relating to infant well being.

Many of the residents on Kansas City’s east side face hardships with employment, housing, and transportation. Most frequently, healthcare gets sidestepped for daily living necessities.
Our Giving Hope campaign supports neighborhood families as they seek medical care from Hope Family Care Center. Our team is committed to providing the best quality healthcare, regardless of the ability to pay.

If your workplace has employee matching options related to charitable donations, please coordinate through your Human Resources Department.
Employee Matching for HCA Employees
Every calendar year, each colleague has $500 in matching funds available for the HCA Healthcare Hope Fund, and $500 in matching funds available for other nonprofit organizations. In order to receive a match, colleagues only have to give $25 to the organization.
